Temporary Community Health Worker
Location: Covering Weymouth, Portland & the West of Dorset
Salary: £17,311.25 (£27,698pa FTE)
Temporary - fixed term for 9 months (25 hours per week)
The first suitable applicant will be appointed.
YOU is a registered charity that has been providing care, support and advice for over 30 years to vulnerable people, working across Hampshire, Portsmouth, Somerset, Dorset, Sussex and the Isle of Wight to give them the tools and resources they need to live independent, secure and valued lives.
Essential Requirements:
Have an awareness of the factors that can put people at risk of homelessness and of the impact this can have on wider health and wellbeing
Have a broad skills and knowledge base including knowledge of housing related support; housing law; debt; welfare benefits; welfare reform and universal credit
Have experience of working with a diverse range of client groups
Due to the nature of the role the Community Health Worker will need to hold a full driving license with access to a vehicle
The Role:

Our service recognises the importance of positive health and wellbeing and aims to support people to maintain their independence and prevent the escalation of problems that could impact on this.
As a Community Health Worker, you will be responsible for:
Managing your own caseload, providing one to one support in a client’s own home or the community.
Supporting clients via drop-ins (local to your area of work)
Providing practical support for example navigating an individual through a housing application, benefits application or budgeting.
Encouraging individuals to gain access to resources that they may not have been aware of that support positive health and wellbeing for example community services, social groups, peer support, addiction services.
Partnership working with a range agencies and organisations, often advocating on a client’s behalf
Regular meetings at the local office in Dorchester.
Travel across Dorset to meet the needs of our clients and service.
You will be passionate about working with people
You will work alongside our clients as an equal partner and give advice and guidance at a level matching the person’s skills and strengths.
Developing positive working relationships with the local community and support agencies to create a network of support for your clients will be central to the work you do.
