Be the First Point of Support That Makes a Difference
We are looking for motivated, compassionate, and customer-focused Community Health Navigators to join our BSW (Bath & North East Somerset, Swindon and Wiltshire) Community Health Services Single Point of Access (SPA) Team.
As a Community Health Navigator, you will play a key role in ensuring patients receive the right care, at the right time, from the right service. Working within a busy and supportive team, you will act as the first point of contact for patients, carers, healthcare professionals, and colleagues, providing a professional, responsive, and compassionate service across a range of community healthcare services.

Location: Unit 7, Bath Business Park, Roman Way, Peasedown St John, Bath, BA2 8SG
